Hi everyone,

What format did Visual Prolog 5x store its databases in? Can you point me to a tool that would allow me to inspect a database from that era?

Such a tool does not exist. A bad thing about such databases is, that it is only possible to read them with a program that have the exact same domain declarations as the program that wrote them.
